What is ketamine-assisted psychotherapy?

Ketamine-Assisted Psychotherapy is an evidence-based psychedelic treatment that combines off-label ketamine treatment with personalized psychotherapy. KAP addresses the root causes of depression, anxiety and PTSD for more meaningful breakthroughs in therapy and long-term improvement in mental health outcomes. Treatment is provided by a licensed KAP provider (i.e., psychotherapist) in conjunction with Journey Clinical’s medical team (i.e., psychiatrists).

A Safe, Powerful Treatment

Ketamine has a long history of safe use in medical settings, including anesthesia and pain management in pediatric populations. When administered in controlled, therapeutic environments it is considered very safe. The doses used in KAP are lower than those used in anesthesia and are carefully monitored by trained professionals to ensure your safety and comfort throughout the process.

Backed by Science

Ketamine has rapidly-acting antidepressant and mood-enhancing effects, which can begin to take effect within 1-2 hours after treatment. It works by blocking the brain’s NMDA receptors as well as by stimulating AMPA receptors, which are thought to help form new synaptic connections and boost neural circuits that regulate stress and mood. Ketamine has also been shown to enhance overall neuroplasticity for lasting symptom improvement.

The Power of the Therapeutic Alliance

In a 2021 study by Mitchell et al, the effect size of MDMA-Assisted therapy for severe PTSD was 2.1 vs. MDMA alone at 0.91, more than double with the inclusion of psychotherapy. A 2021 study by Davis et al found that the stronger the therapeutic alliance, the better the patient outcomes for major depressive disorder.

 Experience KAP with Your Therapist

Once you receive your ketamine prescription, your psychotherapist will schedule time for your KAP preparation, dosing and integration psychotherapy sessions together. A typical course of treatment includes at least 2 preparation sessions, and 6-8 ketamine dosing sessions each followed by an integration session.

 Receive Ongoing Collaborative Care

Schedule follow-up sessions with our medical team to continue treatment. Your therapist and your prescriber will collaborate to support you on your journey. We can also support you with psychiatric medication management in addition to KAP.

What to Expect During Dosing

Ketamine dosing sessions typically last between 2-3 hours. You will take your ketamine in the safety of your home or your psychotherapist's office depending on what they offer. Your ketamine comes in a lozenge form, which you will dissolve under your tongue.

The effects of ketamine, which most patients find pleasant, last for approximately 45 minutes. These effects can make you feel “far from” your body, and facilitate shifts in perception that can often feel expansive in nature. Your motor and verbal abilities will be reduced, so you’ll be lying down in a comfortable position during the experience.

Once these effects subside, your psychotherapist will spend the remainder of your appointment giving you space to process and discuss your experience. While it may feel hard to articulate what happens during the experience, patients feel like the insights gained are clear.

After you dosing session, you will be able to continue on with your day as usual, but will need to avoid driving or operating heavy machinery.

Why Ketamine + Therapy Matters

Psychotherapy with a trusted KAP provider creates the best conditions for faster, longer-term mental health improvements. Many clinical trials have demonstrated that the delivery of psychedelics within the context of psychotherapy drives better outcomes than prescribing psychedelic medicines alone.  In a 2019 study led by Jennifer Dore, patients experienced significantly decreased depression & anxiety scores (HAM-A & BDI at p<0.0001) in private practice setting with a licensed mental health professional vs. receiving ketamine alone.
